Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and assists in developing the therapy plan of care.
• Provides PT services within scope of practice, per plan of care and in coordination with the health care team.
• Observes and reports patient response to treatment and any condition changes to the supervising clinician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Educates patient and family/caregivers about the plan of care, limitations, and progress.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation and frequency of care.
