Optimal Care is seeking a licensed Physical Therapist to provide one-on-one rehabilitation in patients' homes in Jackson, MI. You will assess functional abilities, develop individualized plans, and guide targeted interventions to improve mobility and safety.

Responsibilities include educating patients and caregivers, collaborating with an interdisciplinary team, and traveling daily within the service area. A Master's/Doctorate in PT and current state license are required, with at least 1 year of

Formación

Master's or doctoral degree from an accredited Physical Therapy program.
Current state license as a Physical Therapist.
Minimum 1 year of experience as a licensed Physical Therapist.
Willingness to travel daily for home visits within service area.

Responsabilidades

Assess patients' functional abilities and home environment to develop individualized treatment plans.
Guide patients through interventions to improve mobility, balance, strength, and safety.
Provide one-on-one rehabilitation care in patients' homes and educate caregivers.
Collaborate with an interdisciplinary care team to optimize outcomes and adherence.
Travel daily within the service area to deliver care across homes, facilities, and communities.
