Caretenders, a member of the Optum family, is seeking a Physical Therapist for Home Health in Massachusetts. You will assess patients’ functional status, develop a plan of care, and implement interventions to improve mobility and well-being in the patient’s home.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver’s license and access to reliable vehicle or public transportation.

Responsibilities

• Evaluate patient’s functional status and PT needs; assist with plan of care.
• Provide services within state scope and plan of care.
• Observe and report patient responses to treatment to supervisor or physician.
• Treat patients to relieve pain and restore function per standards.
• Instruct patient and family about plan of care and progress toward goals.
• Visit patients per Plan of Treatment and submit progress notes.
• Complete evaluations and develop PT plan within state guidelines.
• Report outcomes and anticipated frequency of care.
