Job summary

Optum, through Caretenders in Home Health, is seeking a Physical Therapist to assess and treat patients in their homes, focusing on functional status, mobility, endurance, and pain. You will develop and adjust plans of care, perform interventions, and monitor progress in collaboration with the health care team.

Requirements include active Physical Therapy licensure in the state, CPR certification, and a valid driver's license with reliable transportation.

Current Physical Therapy licensure in the state of practice.
Current CPR certification.
Current driver's license and access to reliable transportation.

Responsibilities

Evaluates the patient's functional status and physical therapy needs; assists in plan of care.
Provides services within the scope of practice and plan of care.
Observes and reports patient response to treatment to nurse/physician.
Treats patients to relieve pain, restore function, and maintain maximum functioning.
Instructs patient and family regarding the plan of care and progress.
Visits patients and submits progress notes per policy.
Completes evaluations and develops PT plan of care per state guidelines.
Reports outcomes and anticipated frequency of care.
