Job summary

North Idaho Home Health, part of LHC Group and Optum family, is seeking a Physical Therapist to assess and evaluate patient care needs in home health, develop treatment plans, and implement interventions to improve mobility and well-being.

You will visit patients per plan of treatment, document progress, collaborate with physicians and nursing staff, and ensure care complies with state guidelines. The role emphasizes compassionate care and strong teamwork.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Driver’s license and access to a dependable vehicle.
• Preferred CPR certification or ability to obtain within 90 days.

Responsibilities

• Evaluates patient functional status and therapy needs to inform the plan of care.
• Develops and implements PT plan of care in coordination with the health care team.
• Provides services within state practice laws and plan of care.
• Observes and reports patient responses and changes to supervising staff.
• Educates patient and family about plan of care and progress.
• Documents evaluations and progress notes per policy.
• Visits patients according to treatment plan and tracks productivity.
