Job summary

Arlington Assured Home Health, a part of LHC Group, part of the Optum family of businesses under UnitedHealth Group, is hiring a Physical Therapist for home health assessments and care planning in Washington state.

You will evaluate functional status, develop treatment plans, perform interventions, and monitor progress, while collaborating with a care team and educating patients and families. Competitive compensation includes per-visit and hourly pay components.

Qualifications

• Current driver's license and access to a dependable vehicle or public transportation.
• Current physical therapy licensure in the state of Washington.
• Preferred: CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient functional status and PT needs; assists physician in plan of care.
• Provides PT services within scope of practice and in coordination with the health care team.
• Monitors and reports patient responses to treatment to supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Educates patient and family about plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment; completes progress notes per policy.
• Completes all evaluations and develops PT plan of care per state guidelines.
• Reports outcomes, goals, and projected frequency of care.
• Function in diverse home environments regardless of patient demographics.
