Job summary

Centralia Assured Home Health, a part of LHC Group within the Optum family, seeks a licensed Physical Therapist Assistant to provide treatment and instruction in home health settings in Washington state. You will implement the established plan of care and educate patients to improve well-being while working under a supervising PT.

The role includes coordinating with the health care team, guiding families, and ensuring safe, effective therapy delivery in patients' homes.

Qualifications

• Licensed to assist in the practice of physical therapy in Washington state.
• Driver's license and vehicle insurance or reliable transportation.
• Ability to work with patients of diverse backgrounds and conditions.

Responsibilities

• Follow all state laws governing home health PT practice and the supervising PT's plan of care.
• Instruct patients in active and passive exercises, gait, ADL, and mobility training.
• Observe and report patient responses to treatment to the supervising PT and care team.
• Instruct patients, families, and caregivers on therapy within scope of PTA.
