CHRISTUS Home Health in Texas is seeking a licensed Physical Therapist to deliver in-home rehabilitation services, assess functional status, and develop evidence-based plans of care. You will collaborate with physicians and the health care team to improve mobility, pain relief, and quality of life for patients.

Responsibilities include performing patient evaluations, documenting progress, adhering to state practice guidelines, and educating patients and families about the plan of care.

Formación

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle.

Responsabilidades

• Evaluates the patient's functional status and physical therapy needs; assists the physician in the therapy plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient response to treatment to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient, family/caregivers on plan of care and progress.
• Completes visits and progress notes per policy; develops PT plan of care within state guidelines.
