Job summary

VitalCaring is seeking a Home Health Physical Therapist (PRN) to deliver patient-centered care in patients’ homes across Oklahoma. You will assess, plan, and implement therapy focused on restoring mobility and independence, working with physicians and care teams to ensure seamless transitions.

The role emphasizes autonomy, compassionate care, and ongoing clinical education. A valid PT license in the state and CPR certification are required, with 1–2 years of home health preferred.

Qualifications

• Graduate of an accredited physical therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner closely with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.
