Job summary

VitalCaring seeks a Home Health Physical Therapist to deliver patient-centered care in patients' homes across Texas. You will assess function, design care plans, and provide hands-on therapy to improve mobility and independence.

You’ll supervise PTAs where allowed, educate patients and families, document progress in the EMR, and coordinate with the care team to ensure safe transitions and high-quality outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff
