Job summary

Cherokee Nation Home Health Services is seeking a licensed Physical Therapist to provide in-home therapy services across Oklahoma. The PT will evaluate patients, design individualized plans, and implement gait, balance, and mobility therapies in patients' homes.

The role emphasizes collaboration with physicians, nurses, therapists, and caregivers to promote safe recovery, education, and ongoing progress documentation. A driving license and CPR certification complete the requirements.

Qualifications

• Licensed Oklahoma Physical Therapist.
• Graduate of an accredited physical therapy program.
• Home health experience preferred but not required.
• CPR certification.

Responsibilities

• Evaluate patients' physical abilities and develop individualized treatment plans.
• Provide therapeutic exercises, gait training, balance activities, and mobility interventions.
• Assess home environments and recommend equipment or modifications to improve safety and independence.
• Educate patients and caregivers on treatment plans, exercises, and injury prevention strategies.
• Document patient progress and maintain accurate clinical records in compliance with agency and regulatory standards.
• Collaborate with interdisciplinary team members to coordinate care and achieve optimal patient outcomes.
• Monitor patient responses to treatment and modify care plans as needed.
