Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting. The PRN position offers flexible assignments and competitive compensation while maintaining control of your schedule.

Applicants should hold a Florida PT license and have the ability to work independently within an interdisciplinary team, delivering evidence-based care and thorough documentation.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.

Education

Physical Therapy degree (Bachelor/Master/Doctorate)
