Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy services for residents in home settings around Clearwater, FL. This PRN role offers a flexible schedule and the ability to select assignments that fit your availability.

Qualifications

• Active Florida Physical Therapist license required.
• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.

Responsabilités

• Evaluate and treat hospice patients in home settings.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate documentation in EMR.
