Job summary
Sobe Innovative Rehab in Clearwater, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in a home setting. Qualifications

Responsibilities
- Evaluate and treat residents in an assisted living community and/or home setting.
- Develop and implement individualized treatment plans.
- Collaborate with physicians, families, and the interdisciplinary care team.
- Maintain accurate, timely documentation in the EMR.
