Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide Hospice patient therapy in a home setting. This PRN role offers flexible assignments and the chance to support older adults in maintaining independence.

Responsibilities include evaluating residents, creating treatment plans, coordinating with a care team, and documenting in the EMR. Florida PT license required; new graduates are encouraged to apply.

Qualifications

• Licensed Physical Therapist in FL (PT)
• Masters or Doctorate in Physical Therapy preferred
• New graduates welcome to apply

Responsibilities

• Evaluate and treat residents in a home-setting or assisted living community
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR
