Job summary

Sobe Innovative Rehab in Orlando, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide Hospice care services in patients' homes. This PRN role offers flexible assignments and the opportunity to support independence and quality of life for older adults.

Responsibilities include evaluating residents, developing personalized treatment plans, coordinating with physicians and families, and accurate EMR documentation. Florida PT license is required.

Qualifications

• Active Florida Physical Therapist (PT) license.
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
