Job summary

Lifespan Therapy seeks licensed Physical Therapists to join our home health network in Maricopa County, serving the geriatric population with flexible, independent practice.

Provide skilled evaluations and therapy in patients' homes, focus on functional outcomes, document progress, and set your own caseload. Our app-based platform handles credentials, scheduling, referrals, authorizations, and billing, letting you concentrate on care.

Qualifications

• Valid Arizona Physical Therapy license.
• CPR or BLS certification.
• Valid driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability insurance.
• Business license (if applicable).

Responsibilities

• Provide Skilled Home Care: Deliver individualized physical or occupational therapy services (evaluations and treatments) in the patient's home environment.
• Perform Therapeutic Interventions: Focus on functional strengthening, gait training, ADL retraining, balance, and fall risk reduction.
• Report & Collaborate on Patient Progress: Document patient responses to treatment and maintain accurate records to ensure a coordinated approach.
• Manage Your Caseload: Set your own schedule and visit cadence based on your personal availability and the patient's plan of care.
• Document Efficiently: Use one simple EMR system for all notes (with tools available to minimize documentation time!).
