Job summary

Lifespan Therapy is seeking an experienced Home Health Physical Therapist to deliver patient-centered care directly in the homes of geriatric patients across California. You will conduct comprehensive in-home evaluations and develop personalized plans of care that emphasize mobility, safety, and independence.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver’s license and auto insurance.
• TB test within the last year.
• Professional liability and business license (if applicable).

Responsibilities

• Provide in-home PT evaluations for geriatric patients and develop individualized plans of care.
