Aspire – Compassionate Care is seeking a skilled Physical Therapist (PT) to join our home health team serving Maricopa County, AZ. This role offers part-time opportunities with paid per visit rates starting at $85 per visit, flexible scheduling, and mileage reimbursement to support patient-focused care.

You will evaluate and treat patients in their homes, coordinate with the rehabilitation and clinical team, educate families, and document progress.

Formación

• Active Physical Therapist license in Arizona.
• Minimum 1 year PT experience preferred.
• TB test within 12 months and CPR certification required.
• Valid driver's license and reliable transportation.

Responsabilidades

• Provide effective patient care using approved therapy techniques and procedures.
• Evaluate patient condition and establish measurable treatment goals.
• Develop and implement individualized treatment plans.
• Supervise and coordinate care with PTAs and HHAs.
• Document patient progress, updates, and treatment notes accurately and on time.
• Educate patients and families on exercises, mobility, and use of adaptive equipment.
• Evaluate home environments and recommend modifications to improve safety and independence.
• Ensure all services meet HIPAA standards for confidentiality.
• Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care.
