Our app handles referrals, scheduling, and billing so you can focus on evaluations, treatments, and patient progress using TherapySync EMR. The role supports work‑life balance across the Phoenix metro area.

Qualifications

• Valid Arizona Physical Therapy license required.
• CPR or BLS certification required.
• Driver's license and auto insurance required.
• Physical + TB test within the last year.
• Professional liability insurance required.
• Business license if applicable.

Responsibilities

• Provide Skilled Home Care: deliver evaluations and treatments in the patient’s home.
• Perform Therapeutic Interventions: focus on strengthening, gait training, ADL retraining, balance and fall risk reduction.
• Report & collaborate on patient progress: document responses and maintain accurate records.
• Manage your caseload: set your own schedule and visit cadence based on availability and care plan.
• Document efficiently: use a single EMR system for notes with tools to minimize documentation time.

What You'll Do

• Provide Skilled Home Care: Deliver individualized physical or occupational therapy services (evaluations and treatments) in the patient's home environment.
• Perform Therapeutic Interventions: Focus on functional strengthening, gait training, ADL retraining, balance, and fall risk reduction to help patients reach their independence goals.
• Report & Collaborate on Patient Progress: Document patient responses to treatment and maintain accurate records to ensure a coordinated approach.
• Manage Your Caseload: Set your own schedule and visit cadence based on your personal availability and the patient's plan of care.
• Document Efficiently: Use one simple EMR system for all notes (with tools available to minimize documentation time!).

What You'll Need
