Job summary

Lifespan Therapy is expanding its clinician-first home health platform to Arizona, seeking licensed Physical Therapists to join the Maricopa County network. Enjoy ultimate flexibility and independence with app-based scheduling and streamlined billing, while delivering patient-centered home care.

You will perform evaluations and treatments in patients' homes, focusing on functional goals, gait, balance, and ADL training, with support for documentation and referrals from a centralized team.

Qualifications

• Valid Arizona Physical Therapy license.
• CPR or BLS certification.
• Valid driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability insurance.
• Business license (if applicable).

Responsibilities

• Provide Skilled Home Care: deliver evaluations and treatments in the patient’s home.
• Perform Therapeutic Interventions: focus on strengthening, gait training, ADL retraining, balance, and fall risk reduction.
• Report & collaborate on patient progress: document responses and maintain records for coordinated care.
• Manage your caseload: set your own schedule and visit cadence.
• Document efficiently: use one EMR system for notes.

What You'll Do

• Provide Skilled Home Care: Deliver individualized physical or occupational therapy services (evaluations and treatments) in the patient's home environment.
• Perform Therapeutic Interventions: Focus on functional strengthening, gait training, ADL retraining, balance, and fall risk reduction to help patients reach their independence goals.
• Report & Collaborate on Patient Progress: Document patient responses to treatment and maintain accurate records to ensure a coordinated approach.
• Manage Your Caseload: Set your own schedule and visit cadence based on your personal availability and the patient's plan of care.
• Document Efficiently: Use one simple EMR system for all notes (with tools available to minimize documentation time!).
