Interim HealthCare of Augusta is seeking a Home Health Physical Therapist to provide one-on-one therapy in patients' homes around Roanoke, VA and surrounding areas. You’ll work independently to improve strength, mobility, and daily functioning through individualized plans and hands-on care.

The role requires a Virginia PT license, a graduate PT degree, CPR, and willingness to travel. Part-time/PRN schedule offers flexible hours and mileage reimbursement.

Qualificações

• Graduate of an accredited Physical Therapist program
• Active PT license in the state of Virginia
• Minimum of 2 years of physical therapy experience (home health preferred)
• CPR certification required
• Knowledge of state and federal home health regulations
• Strong clinical judgment, interpersonal skills, and compassion
• Ability to work independently in a home setting
• Willingness to travel throughout the service area

Responsabilidades

• Provide home-based physical therapy to patients recovering from illness or injury
• Conduct patient assessments and develop individualized treatment plans
• Monitor patient progress and modify care plans as needed
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain
• Educate patients on therapy goals, exercises, and self-care techniques
• Coach family members on safely assisting with exercises and patient care
• Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, OT, and SLP
• Maintain accurate and timely documentation of patient progress

Pay: $45-$100 per visit depending on visit type + mileage reimbursement

Job Overview:

We are currently seeking a compassionate and skilled Home Health Physical Therapist – PT to provide personalized, one-on-one therapy in a home setting throughout Roanoke and the surrounding service area. This position is ideal for a Physical Therapist who enjoys working independently while helping patients regain strength, mobility, and functionality in their daily environment.

As a Home Health Physical Therapist – PT, you will work directly with patients recovering from illness, injury, or mobility challenges while developing individualized treatment plans focused on improving patient outcomes and quality of life.
