Interim HealthCare of Salem (Roanoke) is seeking a compassionate Home Health Physical Therapist – PT to provide one-on-one therapy in patients' homes across Roanoke and surrounding areas. This role emphasizes independent work and improving mobility, strength, and daily functioning in the patient's own environment.

Flexible part-time/PRN schedule with per-visit pay $45-$100 plus mileage reimbursement, comprehensive in-home care services, and opportunities for professional growth through online

Qualificações

• Graduate of an accredited Physical Therapist program.
• Active PT license in the state of Virginia.
• Minimum of 2 years of physical therapy experience (home health preferred).
• CPR certification required.
• Knowledge of state and federal home health regulations.

Responsabilidades

• Provide home-based physical therapy to patients recovering from illness or injury.
• Conduct patient assessments and develop individualized treatment plans.
• Monitor patient progress and modify care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain.
• Educate patients on therapy goals, exercises, and self-care techniques.
• Coach family members on safely assisting with exercises and patient care.
• Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, OT, and SLP.
• Maintain accurate and timely documentation of patient progress.

Schedule: Part-Time / PRN (Flexible Scheduling Available)

Pay: $45-$100 per visit depending on visit type + mileage reimbursement

Job Overview:

We are currently seeking a compassionate and skilled Home Health Physical Therapist – PT to provide personalized, one-on-one therapy in a home setting throughout Roanoke and the surrounding service area. This position is ideal for a Physical Therapist who enjoys working independently while helping patients regain strength, mobility, and functionality in their daily environment.

As a Home Health Physical Therapist – PT, you will work directly with patients recovering from illness, injury, or mobility challenges while developing individualized treatment plans focused on improving patient outcomes and quality of life.
