Job summary

Aspire – Compassionate Care is seeking a Physical Therapist (PT) to join our home health team in Maricopa County, AZ. Provide individualized, one-on-one care in patients’ homes, coordinating with the rehabilitation team.

We offer flexible scheduling, per-visit pay starting at $85, mileage reimbursement, 3 weeks of PTO per year for eligible employees, referral bonuses up to $500, and growth opportunities across multiple states; join a supportive, patient-first culture.

Qualifications

• Active Physical Therapist license in Arizona.
• Minimum 1 year of PT experience preferred.
• TB test within 12 months and CPR certification.
• Valid driver’s license and reliable transportation.
• Must meet HIPAA standards.

Responsibilities

• Provide effective home health PT care with individualized treatment.
• Evaluate patient condition and set measurable goals.
• Develop and implement treatment plans.
• Supervise PTAs and HHAs.
• Document progress and treatment notes accurately.
• Educate patients and families on exercises and safety.
• Assess home environments and suggest safety modifications.
• Coordinate with nursing, social work, and other staff.
