Job summary

Sweetwater Care is seeking a licensed Physical Therapist (CA) to provide in-home rehabilitation services. You will conduct patient interviews, develop and implement individualized treatment plans, and monitor progress in a home setting.

Travel to various home care locations is required. The role focuses on functional training, exercise, and related modalities to alleviate pain, with coordination among other healthcare professionals to optimize patient outcomes.

Qualifications

• Be licensed as a Physical Therapist in the employing state.
• At least 1 year of clinical experience preferred.
• Demonstrate knowledge and skill in current therapy practice.

Qualifications:

• Be currently licensed as a Physical Therapist (PT) in the state of employment
• A minimum of one year of clinical experience is preferred
• Demonstrate knowledge and skill in current therapy practice
• Possess a valid state driver’s license and automobile liability insurance
