Job summary

Sweetwater Care is seeking a licensed Physical Therapist to provide in-home patient care across Houston, TX. You will interview patients and families, assess progress, and implement personalized treatment plans delivered in the home environment.

The role requires travel to various home care settings and collaboration with the care team to achieve functional goals. A TX PT license and valid driver's license are required.

Qualifications

• Must be licensed as a Physical Therapist in the state of employment.
• Minimum of one year of clinical experience preferred.
• Demonstrate knowledge and skill in current therapy practice.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Interview patients and family in patient's home.
• Create individualized in-home treatment plans.
