Job summary

Sweetwater Care is seeking a licensed Physical Therapist to deliver in-home care. You will interview patients and family members, create individualized treatment plans, outline goals, and monitor progress in a home setting.

Travel to different home care sites will be required. You will collaborate with other healthcare professionals and maintain a current PT license; one year of clinical experience is preferred.

Qualifications

• Must hold a current Physical Therapist license in the hiring state.
• Minimum of one year of clinical experience preferred.
• Demonstrates knowledge and skill in current therapy practice.
• Valid state driver's license and automobile liability insurance.
