New Solutions is seeking a compassionate Physical Therapist for home health in the Denver area. This full-time role offers personalized care to a diverse client base, helping patients improve health and mobility at home.

The position requires a minimum of nine months of PT experience, a CAPTE-accredited master's degree, CPR certification, and the ability to travel to multiple locations. Community/home health experience is preferred, with comprehensive benefits included.

Qualifications

Responsibilities

• Provides patient care and documents treatment according to physician orders, agency policy and patient needs.
• Informs physician of assessment results and recommended interventions prior to initiating care.
• Develops and documents treatment plans, selects intervention approaches and establishes priorities.
• Delivers organized patient treatment, including advising, educating and counseling the patient.
• Communicates pertinent patient information to physicians and team members; updates status and plan.
• Administers appropriate treatment per patient needs and payer/agency parameters.

We're seeking a compassionate and skilled PT to join our home health team.

This full-time position offers the unique opportunity to provide personalized care to our diverse client base, helping them achieve their health and mobility goals.

Position Responsibilities:

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience
• Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations
