Job summary

Aveanna Healthcare is seeking a Physical Therapist (PT) PRN for home health in Boynton Beach and surrounding areas. You will perform mobility assessments, deliver therapy per physician orders, and collaborate with a care team to support patient recovery in the home setting.

Requirements include a PT degree, an unrestricted license in the state, and valid CPR. Prior home health experience and OASIS familiarity are preferred.

Qualifications

• Degree in Physical Therapy from an accredited university/program.
• Current, unrestricted state license as a Physical Therapist in the state of practice
• Valid CPR

Responsibilities

• Perform initial and ongoing mobility, strength, and functional assessments.
• Deliver PT services per physician orders and plan of care.
• Maintain and document plan of care with progress and discharge planning.
• Collaborate with healthcare team and family to coordinate care.
• Complete OASIS assessments where allowed by state regulations.
• Document accurately to avoid payer denials and ensure compliance.
