Job summary

Aveanna Healthcare is seeking a Physical Therapist (PT) for full-time home health visits in the Brandon area, Florida. You will assess, plan, and deliver therapy in patients’ homes, coordinating with families and medical teams to optimize outcomes.

The role emphasizes accurate documentation, adherence to physician orders, and effective discharge planning with patient education for ongoing home exercise programs. Requires valid Florida PT license and CPR certification.

Qualifications

• Degree in Physical Therapy from an accredited university/program.
• Current, unrestricted state license as a Physical Therapist in the state of practice
• Valid CPR

Responsibilities

• Perform initial and ongoing mobility, coordination, and strength assessments.
• Document plan of care, goals, progress and outcomes.
• Provide therapy services per physician orders in the patient's home.
• Discharge planning and home exercise instruction.
• Coordinate with healthcare team and family, documenting per policy.
• Complete OASIS assessments where allowed by state regulation.
