Job summary

Aveanna Healthcare is seeking a Physical Therapist (PT) to join our full-time home health care team in Florida’s Okeechobee area. You will assess, plan, and deliver in-home therapy to help patients regain mobility and independence following illness or injury.

As part of our multi-disciplinary team, you’ll document progress, coordinate with caregivers, and educate families on home exercise programs, with a focus on compassionate, patient-centered care and compliant practice.

Qualifications

• Degree in Physical Therapy from an accredited university/program.
• Current, unrestricted state license as a Physical Therapist in the state of practice (Florida).
• Valid CPR.

Responsibilities

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
• Delivery of physical therapy services to comply with prescribed physician orders.
• Document plan of care and progress, and determine discharge plans as appropriate.
• Collaborate with healthcare team and educate patients and caretakers on home exercise programs.
• Supervise PTA/OTA as applicable and complete necessary OASIS assessments where allowed.
