Job summary

Qualifications

• Graduate of an accredited PT program.
• Minimum 1 year of physical therapy experience.
• Licensed Physical Therapist in Florida.
• Valid Florida Driver’s License.
• Valid CPR certification.

Responsibilities

• Provide PT services prescribed by a physician in the home setting.
• Perform initial and ongoing assessments and develop plans of care.
• Communicate with physicians, family, and care team.
• Supervise Therapy Assistants and Home Health Aides as appropriate.
• Document care and progress notes and plan for discharge.
• Adhere to privacy and compliance policies.
