Job summary

Qualifications

• Completion of a CAPTE-accredited Physical Therapist education program.
• Florida PT license (or eligibility).
• Current CPR certification; online certification is not accepted.

Responsibilities

• Provide skilled PT services in patients' homes on an intermittent basis.
• Perform initial and ongoing assessments, including PT-related evaluations and documentation.
• Assist in developing and implementing the interdisciplinary care plan for PT.
• Educate patients and families on exercises, device use, and home exercise programs.
• Supervise and collaborate with Physical Therapy Assistants and home health aides as per policy.
• Coordinate with physicians; communicate patient status and changes every 30 days.
