Qualifications

• Graduate of an accredited PT program.
• 1 year experience in Physical Therapy minimum.
• Valid Florida Driver's License.
• Licensed Physical Therapist in the state of Florida.
• Valid CPR certification.

Responsibilities

• Provide PT services prescribed by a physician in the home.
• Perform initial and ongoing assessments and revise plan of care as needed.
• Provide therapeutic treatments and equipment evaluation.
• Advise families and care-team on equipment, HEP, safety, and discharge planning.
• Prepare progress notes and evaluate patient outcomes.
• Supervise therapy assistants and aides as appropriate.
• Follow all policies and protect patient health information.
