Qualifications

• Active Florida PT license.
• Home health experience preferred.
• Strong clinical assessment and therapeutic skills.
• Ability to work independently in a field-based environment.
• Reliable transportation, valid driver’s license, and current auto insurance.
• Excellent communication and documentation skills.

Responsibilities

• Perform comprehensive evaluations and develop individualized plans of care for home health patients.
• Provide therapeutic interventions to improve strength, mobility, endurance, balance, and safety.
• Update and adjust treatment plans based on patient progress and clinical findings.
• Educate patients and caregivers on exercises, safety, and home therapy programs.
• Collaborate with nurses, OTs, SLPs, CNAs, and other team members to ensure coordinated care.
• Complete accurate, timely clinical documentation in compliance with Medicare and agency standards.
