Job summary

Home Health of Wilson, part of LHC Group and a member of the Optum family, is seeking a PRN Physical Therapist to evaluate patient needs, develop a treatment plan, and deliver interventions to improve mobility and function in a home health setting.

You will perform assessments, document progress, and coordinate with nurses and physicians while adhering to state practice guidelines. This PRN role offers flexible scheduling and competitive compensation.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws.
• Observes and reports patient response to treatment.
• Treats patients to relieve pain and restore function.
• Informs patient and family about the plan of care and progress.
• Visits patients according to Plan of Treatment and documents progress.
• Completes patient evaluations and develops the PT plan of care.
• Reports outcomes of evaluation and goals.
