Job summary

Home Health of Wilson, part of the Optum family, seeks a Physical Therapist for a PRN role in its home health program. You will assess functional status, develop and implement treatment plans, and evaluate patient progress to improve well-being.

You will coordinate with nurses and physicians to ensure safe, effective care in patients' homes. Responsibilities include documenting visits, following state guidelines, and communicating outcomes.

Qualifications

• Current Physical Therapy licensure in the state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire (preferred)

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and the plan of care, coordinating with health care team.
• Observes and reports patient response to treatment and any changes to supervising nurse or physician.
• Treats patients to relieve pain, develop/restore function, and maintain maximum functioning per standards of practice.
• Instructs patients and family/caregivers regarding plan of care and progress toward goals.
• Visits patients according to Plan of Treatment; completes progress notes and submits per policy.
• Develops PT plan of care within state guidelines; reports outcomes and anticipated frequency of care.
• Function in diverse home environments respecting patient equality.
