Caretenders Home Health a part of LHC Group is seeking a Physical Therapist PRN to perform patient evaluations, develop treatment plans, and deliver therapies in home health settings. The role focuses on improving mobility, reducing pain, and promoting functional independence across patient populations.

Responsibilities include assessing functional status, implementing interventions per plan of care, documenting progress notes, and coordinating with the health care team.

Qualificações

• Current Physical Therapy licensure in state of practice.
• Valid driver’s license and access to a dependable vehicle, or public transportation.
• CPR certification (preferred) within 90 days of hire.

Responsabilidades

• Evaluates patient functional status and develops therapy plan of care.
• Provides PT services within the scope of practice and plan of care.
• Observes and reports patient responses to treatment to supervising nurse/physician.
• Treats patients to relieve pain and restore or maintain function.
• Instructs patient and family on plan of care and progress toward goals.

Formação académica

Physical Therapy licensure in state of practice

CPR certification (preferred)
