Qualifications

• Licensed VA Physical Therapist (PT) in good standing.
• Graduation from an APTA-approved program or AMA-accredited school.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform mobility, strength, balance, and functional assessments and monitor progress.
• Develop and revise individualized care plans and perform skilled PT interventions.
• Educate clients and families on exercises and safety to promote independence.
• Document observations and interventions accurately using EMR.
