BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist for Harrisonburg, Virginia. The PT will conduct home visits, perform mobility assessments, and implement individualized care plans to promote patient independence.

The role allows PRN or part-time arrangements in a senior living office, serving adult and geriatric clients in Harrisonburg, VA. Strong EMR skills and a VA PT license are required.

Qualifications

• A current VA Physical Therapist license.
• Graduation from an APTA-approved PT program with transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform movement testing and gait assessment, balance and safety checks.
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and update individualized PT care plans including exercises and gait training.
• Assess and revise PT plans with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care to promote independence.
