Job summary

NYU Langone Hospital—Long Island is seeking a Physical Therapist (Per Visit) for Home Health Care on Eastern Long Island. The role provides safe, age-appropriate PT to improve mobility and quality of life, through evaluation, planning, implementation, and re-assessment in patients' homes.

Qualified candidates will hold a NYS PT license and a Bachelor's Degree in Physical Therapy; 1–2 years in hospital/home care settings preferred.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Current NYS Physical Therapy license required.
• 1–2 years Hospital (Inpatient) or Orthopedic (Outpatient) or Pediatric setting preferred.

Responsibilities

• Perform objective functional evaluations and re-assess patients with measurable goals.
• Provide therapeutic age-appropriate physical therapy.
• Document assessments, plans, interventions, and outcomes per DOH and TJC requirements.
• Educate patients and families and provide home exercise programs.
• Collaborate with the health-care team to optimize outcomes.
