NYU Langone Hospital-Long Island is seeking a Physical Therapist for Home Health Care on a per-visit basis in Eastern Long Island. The role focuses on safe, age-appropriate physical therapy to improve functional mobility and quality of life, using clinical expertise from examination through re-assessment.

The successful candidate will hold a NYS Physical Therapy license, a Bachelor's degree, and 1–2 years in hospital, orthopedic, or pediatric settings with strong communication and teamwork

Qualifications

• Bachelor's Degree in Physical Therapy; NYS license/registration required.
• 1–2 years hospital (inpatient) or orthopedic (outpatient) or pediatric setting preferred.
• Excellent communication (oral and written), critical thinking, and customer service skills.

Responsibilities

• Performs objective functional evaluation and re-assessment with tests and measures and sets measurable goals.
• Performs other duties as assigned.
• Supports leadership in achieving quality outcomes and participates in quality improvement initiatives.
• Maintains accurate documentation according to regulatory requirements.
• Re-assesses progress and adapts the plan of care as needed.
• Educates patient and family on functional status and home safety with a home exercise program.
• Collaborates with healthcare team to optimize patient outcomes.
• Implements and adjusts interventions based on patient status and plan of care.
