NYU Langone Hospital - Long Island is seeking an Occupational Therapist Per Visit for Home Health Care in Eastern Long Island. The role provides therapeutic, age-appropriate OT to restore or improve functional abilities and to promote cognitive/behavioral changes, planning care through evaluation, implementation, and assessment.

Qualifications include a NYS OT license, ACOTE‑accredited degree, BLS, and 1–2 years in hospital, orthopedic, outpatient, or pediatric rehab settings.

Qualifications

• Bachelor's Degree in Occupational Therapy from ACOTE-accredited program.
• Current NYS OT license and registration; Basic Life Support.
• 1–2 years hospital inpatient or orthopedic/outpatient/pediatric rehabilitation experience.
• Strong communication, customer service and critical thinking; basic computer skills.

Responsabilités

• Evaluates and assesses patients using tests and measures; sets plan of care with short/long-term goals.
• Implements and modifies interventions based on patient status and plan of care.
• Collaborates with healthcare team to optimize outcomes and goals.
• Educates patient and family on functional status; provides home exercise program and home safety guidance.
• Re-assesses progress and updates the plan of care as needed.
• Documents assessment, diagnosis, prognosis, plan, education and intervention per DOH and TJC requirements.
• Supports leadership to achieve goals and participates in quality improvement initiatives.
