Job summary

Beth Israel Lahey Health is seeking a licensed Physical Therapist to coordinate patient care for home health clients in Massachusetts. You will supervise PTA staff, perform OASIS and clinical assessments, and develop individualized care plans in collaboration with physicians and families.

You will lead preventive and rehabilitative therapies, educate patients, and ensure timely documentation and discharge planning.

Qualifications

• Must hold current Physical Therapist licensure to practice in Massachusetts.
• Active BLS certification required; if not, BILH A/BLS competency is required.
• Graduate of an accredited Physical Therapy program; DPT preferred.

Responsibilities

• Coordinate patient care for assigned caseload including delegation to PTA staff and supervision.
• Accept accountability for patient satisfaction scores and quality of care outcomes.
• Complete ante phsyical assessments and histories; develop patient-specific plans of care.
• Instruct, supervise, and evaluate home health aide care every two weeks as needed.
• Communicate with physicians and team disciplines; participate in case conferences and team meetings.
• Assist in discharge planning and ensure adherence to care plan; document per guidelines.
• Provide education to patients and families; address cultural beliefs and preferences.
