Qualifications

• Current Physical Therapy licensure in the state of Washington.
• Current CPR certification.
• Current driver's license and vehicle insurance, or access to dependable transport.
• Ability to work in diverse home settings with patients and families.

Responsibilities

• Evaluates patient's functional status and therapy needs; assists physician with plan of care.
• Provides PT services within state scope of practice and plan of care.
• Observes and reports patient responses to treatment to supervisor/physician.
• Treats to relieve pain, restore function, and maximize functioning.
• Instructs patient, family, and care team about plan of care and progress.
• Visits per plan of treatment; documents progress notes per policy.
• Completes evaluations and develops PT plan within state guidelines.
• Reports outcomes and anticipated frequency of care.
