Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health through its affiliated St. Francis Medical Center Home Health network.

The role involves conducting assessments, developing and executing a therapy plan, and coordinating with the care team to improve patient outcomes. Responsibilities include evaluating functional status, delivering PT interventions, documenting progress, and educating patients and families.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists in developing the plan of care.
• Provides services within the scope of practice per state laws and plan of care.
• Observes and reports patient responses to treatment to supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patients and caregivers regarding plan of care, limitations, and progress.
• Visits patients according to Plan of Treatment and documents progress notes.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Function across diverse home environments with respect to patient needs and conditions.
