Job summary

Compassus is seeking a licensed Home Health Physical Therapist to provide in-home patient care, plan development, and ongoing evaluation in collaboration with an interdisciplinary team. The role includes implementing treatment plans, conducting assessments, and ensuring compliance with third-party payors while maintaining documentation accuracy.

Evidence-based practice and infection-control measures are mandatory, with emphasis on patient safety, mobility, and caregiver education in a home

Qualifications

• Licensed Physical Therapist in state of employment.
• Two years of acute care or rehabilitation experience preferred.
• Strong communication and documentation skills.

Responsibilities

• Provide home health PT services following physician referral and assessment.
• Develop, implement, and revise Plan of Care in collaboration with the IDT.
• Perform OASIS assessments as appropriate and ensure compliance with payors.
• Educate patients and families on transfer techniques and safety.
• Maintain accurate progress notes and timely documentation.
