Job summary

Compassus is seeking a Home Health Physical Therapist to provide coordinated care across age groups in a home health setting. You will assess, plan, implement, and reevaluate treatment plans in collaboration with the IDT, ensuring compliance with third-party payors and documentation standards.

The role requires a BS in Physical Therapy, an active PT license, and preferred 2 years of acute care or rehab experience.

Qualifications

• Active Physical Therapist license in state of employment required.
• Bachelor's degree in Physical Therapy or equivalent.
• Two years of experience in acute care or rehabilitation settings preferred.

Responsibilities

• Plans, implements, and evaluates patient care plans to restore or maintain well-being.
• Performs initial and ongoing PT assessments, including post-acute evaluations.
• Collaborates with interdisciplinary team (IDT) to coordinate care.
• Documents progress notes and updates Plan of Care with physician input.
• Instructs patients on transfer techniques and safe equipment use.
• Ensures infection control practices and patient safety in home health settings.
