Job summary

Compassus is seeking a Home Health Physical Therapist to provide coordinated care to patients across ages in Indiana. You will plan, implement, and evaluate patient care plans, conduct clinical assessments, and collaborate with an interdisciplinary team to ensure high-quality home health services.

The role emphasizes infection control, documentation, and adherence to the care plan while applying Compassus values and supporting a patient-centered care philosophy in a home health setting.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Two (2) years of experience as a Physical Therapist in an acute care or rehabilitation setting highly preferred.
• Active PT license in state(s) of employment required.
• Current CPR certification required.

Responsibilities

• Provide care utilizing infection control measures to protect staff and patient.
• Prepare and complete a Plan of Care in consultation with physician and care-team members.
• Perform initial and ongoing clinical assessments, including OASIS assessments at appropriate time points.
• Collaborate with the interdisciplinary team to coordinate services and ensure continuity of care.
• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on referrals.
