Job summary

Compassus is seeking a Home Health Physical Therapist to provide coordinated care to patients of all ages, develop and implement care plans, and collaborate with an interdisciplinary team. The role emphasizes infection control, patient/family communication, and adherence to the Compassus values.

Responsibilities include performing assessments, planning and revising care plans, and delivering therapy for neuromuscular, neurological, and orthopedic conditions while guiding patients in transfers

Qualifications

• Bachelor of Science in Physical Therapy is required.
• Two years of PT experience in acute care or rehabilitation setting highly preferred.
• Active PT license and CPR certification required.

Responsibilities

• Provides care using infection control measures to protect staff and patient.
• Communicates effectively with patient/family and care team for coordination.
• Performs initial and ongoing clinical assessments including OASIS at appropriate time points.
• Prepares and revises a Plan of Care in consultation with physician and care-team members.
• Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders.
• Instructs patient/family in transfer techniques and use of equipment such as walkers, wheelchairs, canes, etc.
• Performs other duties as assigned.
